Social Prescribing is about developing Stronger Partnerships, between you, your community and supporting services

What do we do?

We will listen, support and explore ‘what matters to you’ and work with you to create a personal plan highlighting your needs. We can provide support, information and signpost within the local community by linking in with voluntary, community sector organisations and other agencies. We can help you to find support that works for you.

How to access SP Team?

A member of your G.P. Practice Healthcare team can refer you to the Social Prescribing Link Worker. We will then contact you to discuss the referral and arrange to meet you in a place that you feel comfortable, for most people this is at home. Together, we will look at what is important to you and your wellbeing and develop a plan, identifying how you can access local services and activities to help you reach your goals, with further support from the Link Worker if needed.

Simon Marlow, Claire Hayward and Gopalee Patel are the Adult Social Prescribers. They support patients over the age of 25+. You can ask to be referred to the Adult SP Team at your GP Practice – you do not have to be seen by a GP or a Nurse, you are able to ask the Reception Team to send a referral to us with a brief outline of the type of support needed.
